As part of your child's ABA services in Ogden, UT, Nova Supports may incorporate water safety instruction and activities to help develop important safety, communication, and independence skills in and around the water.
Before participating, a parent or legal guardian must review and complete the waiver below.
Please note: If your child has had any seizure activity in the past, you must disclose this and receive a doctor's note in order to use the pool.

Privacy & HIPAA
Nova Supports is committed to protecting your family's privacy. Information submitted through this waiver will be maintained in accordance with HIPAA and applicable state and federal privacy laws. Access is limited to authorized Nova Supports personnel involved in your child's care or program administration.
Participation
Water-based activities involve inherent risks. By signing the waiver, you acknowledge these risks and provide permission for your child to participate in water safety activities as part of their services, subject to the terms outlined in the waiver.
Please ensure that any relevant medical conditions, allergies, or safety concerns have been communicated to your child's clinical team before participation.
