Our intake form must be filled out by a guardian before the authorization for use of the pool. No exceptions.

We are so excited for you to Start Your Nova Swim Journey!

We're excited to welcome your family to Nova Swim. Completing our intake form is the first step toward creating a safe, supportive, and individualized swimming experience for your child.

Every swimmer is different, which is why we take the time to learn about your child's strengths, interests, communication style, sensory preferences, and previous experiences in the water before their first lesson. We don't just show up blindly to your first lesson. We like to take the time to prepare and get to know your child well.

Why We Ask for an Intake Form

Our goal is to provide swim lessons that meet your child where they are. The information you share helps our team prepare before your first visit so we can create a positive experience from day one.

Your responses may help us understand:

  • Your child's swimming experience and current comfort level

  • Water safety concerns and goals

  • Communication preferences

  • Sensory sensitivities or accommodations

  • Medical or physical considerations

  • Behavioral supports that help your child succeed

  • Motivators, interests, and favorite activities

The more we know ahead of time, the better we can personalize each lesson.

Designed for Autistic and Adaptive Swimmers

Nova Swim specializes in autism swim lessons and adaptive swim lessons for children with a wide range of developmental, sensory, physical, and communication needs.

Many of the questions on our intake form are designed to help us understand how your child learns best—not to judge or label them. Whether your child is autistic, has sensory processing differences, developmental delays, Down syndrome, ADHD, cerebral palsy, or another disability, our team uses this information to create a supportive learning environment.

Our adaptive approach allows instructors to build trust, reduce anxiety, and teach water safety in ways that are meaningful for each swimmer.

What Happens After You Submit the Form?

Once your intake form is received, our team will review your information and match your family with the appropriate instructor or program whenever possible.

We'll then contact you with the next steps, which may include:

  • Scheduling your first lesson

  • Answering questions about our programs

  • Discussing goals for your swimmer

  • Reviewing any accommodations or support needs

Helping Every Autistic Child Build Confidence in the Water

Learning to swim is about much more than strokes and techniques. Building confidence in thw water and increasing independence can decrease the risk of drowning. By developing life-saving water safety skills your family and ours at Nova Swim can help children discover that the water can be a place of success and joy.
